Health Requirements for School Enrollment
The Code Of Virginia requires that all students enrolling in
school provide proof of meeting the required school health entry
requirements (immunizations and physical exam) before starting.
Parents are responsible for providing documentation that all the
required immunizations are up to date, or that a medical or
religious exemption has been obtained. There are some new changes
which are listed below.
IMMUNIZATION:
* A second dose of varicella vaccine (chicken pox) is required
before entering kindergarten.
* A second dose of mumps vaccine is required before entering
kindergarten.
* Four doses of polio vaccine are required, with one dose being
administered after the 4th birthday. (kindergarten- 12th)
* Four doses of DTaP vaccine are required, with one dose being
administered after the 4th birthday. (kindergarten- 12th)
PHYSICAL:
* Every student is required to have a Virginia School Health
Physical performed within 12 months of school entry.
* A child that attended preschool and has a Virginia School Health
Physical that was done within 12 months of school starting may be
allowed to use that physical. If it is older than 12 months
(August- August) a new physical is required.
The school district has the right to
exclude children from school or prevent them from enrolling if these
health requirements are not met. It is strongly advised that
you call your doctor or health department to make an appointment as
soon as possible to get these requirements completed.

KINDERGARTEN IMMUNIZATIONS

5TH GRADE TDAP VACCINATIONS

***6th grade students are required to have tetanus, diphtheria,
pertussis (Tdap) booster immunization prior to school entry if at
least five years have passed since last shot.
REMINDERS for MEDICATION ADMINISTRATION
* All medicine must be kept in the original container.
*Please don't send medication to school by the child. A parent or
guardian needs to bring the medication to school.
*A physician's authorization is required for all prescription
medication.
*A parent/ guardian's signature is required before medication will
be administered.
* Medicine must be picked up at the end of the school year; any
medicine not picked up will be discarded.
*The Authorization to Administer Medication form is only good for
one school year.
*Any changes in medicine dosage must have a physician order. Each
medicine requires its own form.
* Non-prescription medication will be given according to the
directions on the box unless accompanied by a physician's order
instructing to do differently.
* Acetaminophen and emergency use antihistamine are available in the
school clinic providing there is written permission on file (form
may be obtained from school nurse).
